It is imperative that we be able to reach you in a timely manner while your pet is in the building today. Please have your cell phone listed below and be free to talk.

Please note, if any evidence of fleas is seen on your pet Central Animal Hospital will apply a treatment at your expense.

Has your pet been fasted for 12 hours prior to surgery? If you are filling out this form several days in advance please take note that your pet will need to be fasted for 12 hours prior to their procedure. *


Any additional surcharges required (i.e. gastropexy, cryptorchid, hernia repair, etc.) are excluded from the Healthy Start discount.
 



The following must be completed for you to receive the Healthy Start discount:
 

Pre-anesthetic blood testing: Our greatest concern is the well-being of your pet. Before putting your pet under anesthesia, a blood panel (CBC/Chemistry screen) will be performed to help reduce many of the risks of surgery. A pre-anesthetic coagulation screening test (PT/APTT) can be added on at additional cost but is not necessary for the discount.

In regards to the statement above, please choose one of the below: *

Intravenous Fluids: IV fluids are mandatory for all canine & feline spays, canine neuters and other major surgical procedures. Administration of intravenous fluids will help to maintain your pet's blood pressure and increase his/her circulation during the anesthetic. It will also help him/her to recover more quickly. The intravenous catheter is also used to administer medication if needed.

Microchips help other clinics and organizations identify your animal so that they can return your pet to you should they get lost. Please choose one of the options below: *

Retrovirus Testing: Cats with FeLV or FIV infections may live for many years and pass the infection to other cats. Retrovirus testing can diagnose infection (not just clinical disease). *



After Surgery Wear: To prevent licking/chewing of the incision, a cone collar is provided with the spay/neuter package. An after surgery suit is also available for purchase separately (prices vary by size).
